Completing the Initial Contact Form is the first step in connecting with the DSS office.
This form is to be used by Oakland University students interested in or requesting disability academic accommodations, academic adjustments for temporary injuries/conditions and/or housing accommodations.

All information supplied through this form is protected under FERPA and managed by DSS separately from other university record system.
If you do not have a formal diagnosis, please indicate this in the space below and describe your current challenges.
All Housing requests, including single room requests and Emotional Support Animal (ESA) requests, require our provider form to be filled out. Please use appropriate DSS form ESA Request Form or Housing Request Form and attach it below before submitting your application.
All documentation must contain the following: diagnosis; describes the limitation(s) and/or impact(s) of the condition(s); and outlines how the accommodations directly reduce or alleviate those condition(s), limitation(s), and/or impact(s).
